- Your AMCAS Application;
- UW MSTP preliminary application;
- UW MSTP supplemental application;
- Application fee of $35;
- Letters of recommendation from at least 3 faculty/research mentors submitted through AMCAS;
- If non US Citizen, proof of permanent residency status.
