- Three letters of reference from individuals who are familiar with your background, work, and preparation for a PhD program.
- A current CV.

- A course-by-course evaluation with GPA conversion for all higher institutions attended outside the US.
- If applicable self-report unofficial English Language Proficiency (TOEFL or IELTS) test scores.
